Change of Custodian of Personal Health Information

Northern Industrial Supply Clinic wishes to inform its patients and the public that, effective August 1st, 2025 there will be a change in the custodian of personal health information in accordance with the Personal Health Information Act (PHIA) of Newfoundland and Labrador.

​

The current custodian, Brittney Collins, BScN RN, will be succeeded by Amanda Guy, RN. The new custodian will assume full responsibility for the custody and control of all personal health information collected, used, and stored by Northern Industrial Supply Clinic.

This change does not affect your access to your personal health information or your rights under PHIA. All personal health records will continue to be safeguarded in compliance with privacy and confidentiality obligations under the Act.

In 2024, Northern Industrial Supply established NIS Clinic in Labrador West to provide occupational health services. Our clinic is verified within Alcumus, formerly known as cognibox, which enables us to offer a comprehensive range of services for site access to the surrounding mines. Our licensed health professionals are highly trained by DATAC in DOT and non-DOT specimen collection to provide drug testing. NIS Clinic strictly adheres to the latest CSA and NL regulations to ensure that we provide the most current and best practice testing. Additionally, NIS Clinic collaborates with Main Street Medical Clinic to provide testing services.

Services

Miner's Medical/Pre-Employment Medical

The NIS Clinic's collaboration with Main Street Medical Clinic/Medicuro ensures that essential assessments for issuing initial and repeat miner's medical certificates are conducted in accordance with the NL Silica Code of Practice. This partnership provides comprehensive evaluations, including physical examinations, respiratory questionnaires, occupational history assessments, pulmonary function tests, audiograms, and chest X-rays when necessary, promoting the health and safety of miners in their work environment. A fitness certificate is always issued during a medical evaluation. Please note that employees may be declared fit, fit with restrictions or unfit based on medical assessment. In keeping with PHIA, employees' health information is not to be shared or disclosed to employer without written consent from the employee.

Qualitative Respiratory Fit Test

NIS Clinic provides quantitative mask fit testing as per the CAN/CSA Standards Z94.4-18 for various models of N95, Half and Full Face respirators. All QNFT provided by NIS Clinic is recognized by Rio Tinto/IOC, Tacora Resources, and Arcelormittal. Please note NIS is providing respirator fit testing ONLY. NIS Clinic is NOT a Respiratory Program Administrator. A pulmonary function test, physician review and a respirator certificate may be required as part of QNFT.

Drug and Alcohol Test

NIS Clinic personnel are certified as DOT Specimen Collectors and DOT Breath Alcohol Technicians, meeting the standards set forth by the Drug and Alcohol Testing Association of Canada (DATAC) and in compliance with 49 CFR Part 40 (§40.33). Utilizing Evidential Breath Alcohol Units and Health Canada-recognized drug test kits, the clinic offers Vericheck 10-panel POCT oral fluid or urine drug tests and Quantisal Lab-Based oral fluid drug tests. In cases where an instant drug test yields a non-negative result, a lab-based sample MUST be collected for further testing, with a Medical Review Officer involved for overseeing the lab-based process. A Medical Review Officer (MRO) is a person who is a licensed physician and who is responsible for receiving and reviewing laboratory results generated by an employer's drug testing program and evaluating medical explanations for certain drug test results. A MRO provides quality assurance review of the drug testing process for the specimens under your purview, determine if there is a legitimate medical explanation for laboratory confirmed positive, adulterated, substituted and invalid drug test results, ensure the timely flow of test result and other information to employers and protects the confidentiality of the drug testing information. (DOT, 2023). A Designated Employer Representative (DER) must be established to release and perform drug testing to employees. A DER overlooks elements of occupational safety in the workplace, these individuals’ responsibilities can include booking, billing, training, and/or enforcing negative and positive test result procedures. See form attached on the bottom to fill in information required for DER. Post Accident/Incident/Reasonable Cause Testing: NIS Clinic can provide testing for these scenarios. A supervisor MUST accompany employees to the clinic. Both drug and alcohol testing should be conducted within two hours if possible. If the timelines are stretched for some reason, you must perform the alcohol test within eight hours and the drug test within 32 hours following the incident, as per the Canadian Model for Providing a Safe Workplace. A failure to complete the test within those time frames may lead to the relevance of the results being diminished or questioned (Surehire, 2022).

The 3M™ E-A-Rfit™ Dual-Ear Validation System measures the effectiveness of the earplug from inside the employee’s ear, providing accurate, quantitative results. Assesses fit and compliance of employee chosen hearing protection. 3M Eyewear Fit System may be used as one part of a comprehensive eyewear testing program. It is designed to help assess different sealed eyewear and assess the best fitting pair for individuals.
